Yocto SDK uses some environment variables that are not load in Qt-creator by default, It is necessary to add these parameterswhen program starts up.
Close Qt-Creator and edit the following file: ~/QtSDK/QtCreator/bin/qtcreator.sh. Add the following bold lines at the begining and save changes:

Now, add these variables to SDK you should run qtcreator.sh script to start up Qt-Creator.
